• PPO (Preferred Provider) - accepted my 90-95% of doctors and hospitals nationwide. Offers the broadest network of coverage. No referral needed to see a specialist
• HMO (Health Maintenance) - Usually accepted within the state, more restricted network of doctors and hospitals. Requires Referral for Specialist visits. 
• EPO (Exclusive Provider) - extremely limited network of doctors and hospitals. Most times, individuals will need to travel to get coverage.

This is the amount that you would pay before the insurance company pitches in towards major claims. Most plans will offer Copays after deductibles are met

MAXIMUM amount that you will pay before the policy covers you at 100%
